Permanent Injury

Once per level, when a character would otherwise die, they may elect to instead sustain a permanent injury. Permanent injuries can only be healed by a greater restoration or wish spell or by completing the relevant Downtime Job.

Injury Effect
Bad Shoulder Disadvantage on Dexterity saving throws.
Cracked Ribs When you take the Dash action, you take 1d6 piercing damage.
Fractured Skull Disadvantage on Wisdom saving throws.
Irregular Heart Disadvantage on Constitution saving throws.
Limping Speed reduced by 5 ft.
One-Eyed Disadvantage on attack rolls against targets beyond 30 ft.
Phantom Pain When you take damage, take 1d6 extra damage of the same type.
Spinal Injury Carrying capacity halved.
Trauma Disadvantage on initiative rolls.
Tremors Disadvantage on attack rolls that don't use Strength.
