Permanent Injury
Once per level, when a character would otherwise die, they may elect to instead sustain a permanent injury. Permanent injuries can only be healed by a greater restoration or wish spell or by completing the relevant Downtime Job.

| Bad Shoulder | Disadvantage on Dexterity saving throws. |
| Cracked Ribs | When you take the Dash action, you take 1d6 piercing damage. |
| Fractured Skull | Disadvantage on Wisdom saving throws. |
| Irregular Heart | Disadvantage on Constitution saving throws. |
| Limping | Speed reduced by 5 ft. |
| One-Eyed | Disadvantage on attack rolls against targets beyond 30 ft. |
| Phantom Pain | When you take damage, take 1d6 extra damage of the same type. |
| Spinal Injury | Carrying capacity halved. |
| Trauma | Disadvantage on initiative rolls. |
| Tremors | Disadvantage on attack rolls that don't use Strength. |
